For those of you needing an extended payment program, our office offers CARE CREDIT financing options upon approval.

If you have dental insurance, we want you to receive the full benefit of it. Our office staff can assist you in completing your insurance forms and verifying the coverage of your insurance payment–another service to you. This means that you are responsible for your deductible and the portion the insurance does not cover when you see the doctor. Remember, however, that you are responsible for the account if the insurance company, for any reason, does not honor their commitment to you and to us.
Membership Specials
How your plan works.
- If you don’t have insurance, that’s no problem.
- The 12 month membership fee is due in full upon joining.
- Save 10% on your initial membership fees when you join as a family.
- Membership is effective the 1st of the month of which payment is received.
- Payments are due when services are rendered.
- Pay an annual fee and receive two regular cleanings and x-rays.
- Get an additional discount on all other dental work needed.
- It is the sole responsibility of the member to maximize their benefits by arranging all the appropriate appointments within the 12-month membership period.
- If the appointments are not used during this 12-month period, the member will not be entitled to a refund.
- Renewal payment is due the beginning of the same month of each year.
- The anniversary month of your membership is the month that you originally joined, regardless of when payment was received.
What are the exclusions?
- Our membership is NOT a dental insurance plan. It can only be used here at Eagle Dental Center.
- Membership is non-transferrable and non-refundable.
- It cannot be combined with any other dental insurance coverage or discount.
- Discounts under your membership do not apply to any treatment prior to joining or after the 12-month membership period.
